Getting Started
If you were impacted by the Hermits Peak/Calf Canyon Fire, the deadline to submit a Notice of Loss was March 14, 2025. No new claims may be started, but claims submitted before March 14, 2025 will continue to be processed. For more details on the claims process, please visit our Progress Your Claim page.

A Proof of Loss (POL) form is a detailed account of your loss which will include documentation to support the estimated amount of damage you experienced as a result of the Hermit’s Peak/Calf Canyon Fire.
Additional Claims Office Forms

If you are not satisfied with the compensation amount offered, you can submit a Notice of Appeal (NOA) form. This form initiates the appeals process. For more information on the appeals process, please visit our File an Appeal page.

If you wish to manage your claim directly without attorney involvement, you may complete and sign the Intent to Proceed Without Participation of Counsel form. This allows you to communicate directly with the Claims Office without affecting your relationship with your attorney or authorized representative.

If your property sustained smoke and ash damage due to the Hermit’s Peak/Calf Canyon Fire, you may be eligible for compensation. This form helps you detail the smoke and ash damage to structures, personal property, vehicles, and more, and is required after submitting a Notice of Loss.

Standard Rate Calculators

The Claims Office has developed a standard rate calculator for claimants to calculate different types of costs sustained or incurred because of the Hermit’s Peak/Calf Canyon Fire.
