Key Documents

Checklists

  • Claims Office Line-Item Checklist
This non-exhaustive list contains a host of items, including different types of losses and other fire damage related items, that you may wish to check against. The Claims Office developed this checklist to assist with conversations you are having with your Navigator as you process your claim.
  • Claims Documentation Checklist
When filing a claim, documentation can be one of the most important steps to complete your Proof of Loss form. To simplify and expedite the process, the Claims Office created checklists for the most common types of loss.

Forms

The Notice of Loss (NOL) form signals interest in beginning the claims process. The NOL collects contact information, a general description of losses, and information about other benefits such as insurance, grants, or loans.

The Proof of Loss (POL) form is a detailed accounting of your losses which will include documentation to support the estimated amount of damage you experienced as a result of the Hermit’s Peak/Calf Canyon Fire.

Claims Process Overview

  1. Current Step 1: Submit a Notice of Loss (NOL)
  2. Step 2: Gather Documentation to Support Your Proof of Loss (POL)
  3. Step 3: Receive a Letter of Determination (LOD)
  4. Step 4: Accept or Appeal Your Offer

Step 1: Submit a Notice of Loss

To start your claim, submit a Notice of Loss (NOL) to the Claims Office. Submitting an NOL initiates the claims process, informing the Claims Office that you are seeking compensation for damages from the Hermit’s Peak/Calf Canyon Fire or subsequent flooding.

*All NOLs must be submitted by December 20, 2024, per the Hermit’s Peak/Calf Canyon Fire Assistance Act.

What is an NOL?

The NOL collects claimant contact information, a general description of losses, and information about other benefits such as insurance, grants, or loans. Once an NOL is submitted, the Claims Office assigns a Navigator to support the claimant through the entire process.

How do I obtain an NOL?

How do I Submit a Completed NOL?

What Happens After I Submit an NOL?

The Claims Office will send you a letter of acknowledgement after receiving your NOL. You should receive an acknowledgement letter within two weeks of submitting your NOL. If you do not receive acknowledgment within two weeks, you should reach out to the Claims Office.
